Parents must fill out our intake application and provide all supporting documents (listed on paperwork). The application can be requested to be emailed, faxed or mailed to you.
Parents must bring application to a scheduled appointment with Coordinator. Please call to set up after completing the application. There is a $25 fee for the intake and if you miss this appointment without calling, you will be charged. The intake appointment generally takes 45 min to an hour. We ask that you do not bring your child(ren) with you at this time. The conversation may not be appropriate for them. We encourage you to bring your child another time before the services start, so they can see the visiting room and meet staff. This will need to be set up with the Coordinator.
Both parents will need to go through this process before we can set up services. We do not contact parties to begin this process, they will need to contact us. All contacts are recorded for any future court proceedings.
Once the intakes are completed, we will coordinate the scheduling of your services. We try to adhere to court orders but may not be able to, due to availability of time and staff.
